Hypertermi och malign hypertermi

Definition av hypertermi: Kroppsterperatur >36.5-37.6°C till följd av störd temperaturreglering. Hypertermi uppkommer om värmeproduktion eller värmetillförsel överstiger värmeavgivning.

Vid hypertermi uppstår ett inflammatoriskt tillstånd som liknar SIRS (Systemic Inflammatory Response Syndrome). Temperaturer >40°C är potentiellt farliga och kan leda till hjärtstopp. Hypertermi graderas enligt Tabell 1 nedan.

Kroppstemperatur bör sjunka med 0.1°C/min vid nedkylning.

Malign hypertermi

Malign hypertermi kan uppstå i två situationer:

  • Vid genetisk mutation i RYR1-receptorn, som leder till en konstant (oupphörlig) muskelkontraktion vid tillförsel av halogenerade anestesmimedel.
  • Som en idiosynkratisk reaktion på MDMA (ecstasy) eller amfetamin.

I båda dessa fall uppstår en konstant muskelaktivering med uttalad värmeproduktion, acidos och utveckling av hyperkalemi. Tillståndet är livshotande och kräver omedelbar sedering och behandling med dantrolen.
